Program name: Physio+ Patient Alerts.
Program description: Physio+ is a doctor of physical therapy practice serving Lindale and Tyler, Texas, with three clinic locations and remote care subscriptions. We send SMS messages only to patients and prospective patients who have explicitly opted in during scheduling, intake, contact, or audit booking. Messages may include appointment reminders, rescheduling and cancellation notices, treatment plan updates, exercise and home program reminders, lab or imaging results when applicable, care instructions related to your active plan, and account or scheduling notifications.
Consent collection process
Physio+ uses a clear, two-step approach to obtain consent for SMS communications:
Step 1: Informational disclosure. On every form that collects a phone number, we display a clear statement that text messages may be sent, with a link to this SMS Communication Consent page and to our Privacy Policy.
Step 2: Active opt-in. A separate, unchecked checkbox appears next to the phone number field. The visitor must actively check the box to opt in. The disclosure language appears directly below or beside the checkbox and includes message type, frequency, data rate notice, and opt-out instructions.
Consent is never assumed. The checkbox is never pre-selected. Checking the box is never required to complete scheduling, contact, or any other core action on our website.
Message frequency and data rates
Message frequency varies based on your active care plan and scheduling activity. Message and data rates may apply. Check with your mobile carrier for details about your text and data plan.
How to opt out
To stop receiving messages, reply STOP to any message you receive from us. After you send STOP, we will send you a single confirmation message, and you will no longer receive text messages from Physio+. To resume messages later, opt in again by submitting a new form or replying START to a confirmation message.

Supported carriers
Our SMS program is compatible with all major US carriers. Carriers are not liable for any delayed or undelivered messages.
Privacy and third-party sharing
Physio+ does not share, sell, or rent your mobile phone number or SMS opt-in consent to any third parties or affiliates for marketing or promotional purposes. All the above exclude text messaging originator opt-in data and consent; this information will not be shared with any third parties.
We may share information with our SMS platform provider strictly for the purpose of delivering the messages you have opted in to receive. That provider is bound by appropriate confidentiality and security obligations.

Consent is not a condition of service
Your consent to receive text messages from Physio+ is not a condition of receiving physical therapy care, scheduling an appointment, or using our website or services in any way. You may decline SMS messaging at any time and still access all of our services.
